Job Description

Center for Life Management (CLM) provides the area's most comprehensive array of mental health services. Our Integrated Care Program is an innovative approach to whole healthcare that encompasses the integration of treatment modalities for mental and physical health needs. Working together with a multi-disciplinary team, the Wellness Coach , a cross-trained mental health counselor and certified personal trainer, provides outpatient community-based support to assist client’s with improving symptoms, behaviors, and co-morbid disabilities. This is a full time, 40 hour per week position, Monday-Friday based out of our Derry, NH practice. The schedule will be Monday - Friday; 8:30am - 5pm.

Major Functional Areas and Description of Duties

  • Provide Functional Support Services (FSS) in the home and community-based settings as deemed appropriate for the objectives, interventions and desired outcomes identified in the client’s treatment plan;
  • Engage client in the process of working toward and reaching their personal health, wellness and recovery goals by using motivational strategies and interventions;
  • Engage client in applicable assessments to monitor symptoms and social determinants of health;
  • Provide crisis intervention services, as needed, when clients experience acute exacerbation of symptoms to ensure their safety within the home and/or community;
  • Work closely with treatment team, integrated care programs, natural supports, and external providers to ensure a wrap-around approach is provided for continuity of client care;
  • Support Wellness Coach Team Leader in cultivating partnerships within the community to identify supplemental resources to offset program costs;
  • Provide set hours of billable service each week (eligible for incentive pay upon exceeding productivity expectation);
  • Serve on appropriate committees, both internal and external, as determined by supervisor;
  • Function professionally and in a manner that protects the integrity, confidentiality and rights of all patients;
  • Execute all required services and timely documentation in accordance with agency policies and federal/state standards;
  • Maintain professional memberships, licenses, accreditation, certifications, etc.;
  • Represent CLM in a professional and appropriate manner in all settings;
  • Perform all other duties as assigned by supervisor.

Qualifications, Skills and Experience

  • Bachelor’s degree required in Health Science, Social Work, Rehabilitation, Psychology, Education, Nursing or related field;
  • Personal training, nutrition and/or other health related certifications preferred;
  • A local gym membership required;
  • Ability to demonstrate and explain proper nutrition practices, physical fitness activities, health and wellness techniques to address whole health and wellness goals;
  • Knowledge in motivational interviewing and strategies for building sustainable motivation toward behavior change;
  • Ability to use sound judgment in routine, urgent, and emergent situations;
  • Proficient computer skills, specifically with all Microsoft office applications;
  • Strong ability and desire to work as part of a clinical team while also able to be self-directed and motivated with minimal supervision;
  • Maintain good health and engage in physical activity;
  • Promotes the holistic benefits of good health and a commitment to decreased health disparity and social stigma for individuals with mental illness;
  • Strong interpersonal and time management skills to build professional relationships with clients and manage schedule to accommodate appointments and documentation of services rendered;
  • Effective communication skills, organizational skills, and ability to multi-task.

Physical Demands and Working Conditions

The majority of the work will be visiting clients in the community and also in their homes. Occasional office based and/or remote work required via telehealth. Manual dexterity is needed to use computers. The ability to sit or stand for extended periods of time and lift up to 25 pounds is required. Reliable transportation, a clean driving record, and proof of auto insurance coverage is required.

The Center for Life Management has partnered with New England College to offer a Master’s Degree! This is an exciting opportunity for CLM staff to earn a  Master of Science in Clinical Mental Health Counseling  (MS) degree in 3 years. 

Who’s Eligible and What’s Required:

  • Must be a regular, full-time CLM employee in good standing for 9 months or more.
  • Must hold a Bachelor’s Degree.
  • Must agree to a five (5) year commitment of full-time employment in good standing with the Center for Life Management (beginning from start date/year of Professional Development Program participation).
  • Participants must maintain a B average to continue participation in the program.
