Position Overview:

The Registered Dietitian Nutritionist will be responsible for providing personalized nutrition assessments, counseling, and education to patients with various health conditions and dietary needs. The RDN will work closely with physicians, nurses, and other healthcare professionals to develop and implement effective nutritional plans that align with patients' health goals, medical conditions, and lifestyle preferences. This role requires both clinical expertise and a compassionate, patient-focused approach to care.

Key Responsibilities:

  • Patient Assessment & Counseling:
    • Perform comprehensive nutrition assessments for patients based on their medical history, lab results, and dietary habits.
    • Develop personalized nutrition plans and provide counseling to patients on managing chronic conditions, weight management, and healthy eating.
    • Educate patients and their families on healthy eating practices, special diets, and the relationship between nutrition and health outcomes.
  • Collaboration with Healthcare Team:
    • Collaborate with physicians, nurses, and other healthcare providers to ensure cohesive patient care and treatment plans.
    • Participate in patient care meetings and provide input on nutritional care as part of an interdisciplinary team.
  • Monitor and Follow-Up:
    • Track patient progress and adjust nutrition plans as necessary to achieve optimal health outcomes.
    • Conduct follow-up appointments to assess compliance, address challenges, and refine dietary plans.
  • Nutritional Education & Wellness:
    • Provide educational seminars and materials on nutrition-related topics to patients, staff, and the community.
    • Support wellness programs at Altheda Medical Center by promoting healthy lifestyle habits and preventive care strategies.
  • Documentation & Compliance:
    • Maintain accurate and thorough documentation of patient interactions, progress, and nutritional plans.
    • Stay updated on current nutrition science, industry trends, and best practices to ensure evidence-based care.
    • Ensure compliance with all state and federal regulations related to nutrition care.

Qualifications:

  • Education:
    • Bachelor’s or Master’s degree in Dietetics, Nutrition, or related field.
    • Completion of an accredited dietetic internship program.
  • Licensure/Certification:
    • Registered Dietitian Nutritionist (RDN) credentialing through the Commission on Dietetic Registration (CDR).
    • State licensure as a Dietitian (as applicable).
  • Experience:
    • Minimum of 1-2 years of experience in clinical nutrition or outpatient counseling (preferred).
    • Experience with managing patients with chronic conditions such as diabetes, cardiovascular disease, and obesity is a plus.
  • Skills & Competencies:
    • Strong knowledge of medical nutrition therapy and evidence-based practices.
    • Excellent interpersonal and communication skills, with the ability to motivate and engage patients in making long-term dietary changes.
    • Ability to work both independently and as part of a collaborative team.
    • Proficient in using electronic medical records (EMR) and Microsoft Office Suite.
