Full-Time - Part-Time or Per-Diem LPN Float

May include nights, weekends and holidays

Shift Differential may be available

Duties:

• The LPN provides a full range of hospice nursing care to patients and families in the Home Care Program under the supervision of a hospice RN as a supplement to the care provided by the RN. 
• Provides personal care such as bathing
• Monitors patients' medical conditions and provides accurate reports to attending physicians as appropriate and to the patients' RN.
• Works in an interdisciplinary care setting to teach, assist, and support patients/families with the dying process. 
• Covers On-Call as needed
• Carries out the pain management and symptom management plan to keep the patient as comfortable as possible. 
• Reassesses pain and notifies the registered nurse when pain is not manageable on the current medications.
• Responds to the physical, mental, social and emotional needs of terminally ill patients and their families. 
• Provides care within all service areas of Hospice of Huntington, Inc.

Minimum Qualifications:

• License to practice by the West Virginia Board of Examiners for Licensed Practical Nurses.
• Must also possess a license in Ohio or application must be in progress.
• Minimum of 1-year patient care experience; hospice or home health preferred.
• Proof of liability insurance.
• Current driver’s license, automobile insurance, and reliable transportation.
• Philosophy is compatible with the hospice concept.
• Demonstrates flexibility in scheduling.
• Possesses the ability to travel to all of Hospice of Huntington’s service area.
